VERY GOOD DISCUSSION.

Also, if the lady is very weak, tell her that we need to admit her and give intravenous fluids and observe her until she is strong again. Also ask her to take frequent meals and ask her to set a target weight which is appropriate for her height. Mention to include family and friends.

Any comments???

frankly speaking its not as simple as it seems as many i times they do not feel the need to enroll for support groups n feel they r normal n need help.

family support is required n one needs the person to gain the trust in what the doc is saying

heres a small matter i compiled frm all info above(by sajida) and elsewher. Do add more points as u read on.

Counselling Anorexia Nervosa

Hello, Miss Smith , I am Dr.A, working in this department. I gathered that u had certain concerns regarding your condition which is diagnosed as Anorexia Nervosa. are you anxious? feel free to ask me any doubts or fear you hav regarding this condition.

What exactly is anorexia nervosa?

Anorexia nervosa is an eating disorder. Here, you might feel that you are extremely overweight and try to loose weight by restricting your diet or get rid of the eaten food, say by using laxatives or even, vomiting. This can cause serious life threatening complications.

Wat complications, doc?

Well, Miss Smith, you can get several health problems caused either by undereating or by the methods used to get rid of eaten food (vomiting, excess laxatives, etc).

Problems that may occur include the following.

-Your menstrual periods can stop.

-You can have body chemical imbalances caused by repeated vomiting or excess use of laxatives. For example, a low potassium level which may cause tiredness, weakness, abnormal heart rhythms, kidney damage and convulsions.

-Low calcium levels can lead to tetany (muscle spasms).

-Thinning of the bones (osteoporosis) caused by calcium and vitamin D deficiency. This can lead to easily fractured bones.

-Bowel problems may occur if a lot of laxatives are taken. Laxatives can damage the bowel muscle and nerve endings. This may eventually result in permanent constipation.

-Swelling of hands, feet and face (due to fluid disturbances).

-Teeth problems caused by the acid from the stomach rotting away the enamel with repeated vomiting.

-Anaemia.

-Depression.

-Delayed puberty, stunted growth and delayed physical development (if anorexia occurs in younger age groups.)

-Loss of sex drive (libido).

How can i come out of this?

You need to build up on ur diet and gradually change ur eating habits, so that you finally attain a weight which is desired for your age.

If u think it is impossible for you to achieve this, we can refer you to a specialist mental health team. Members of the team may include psychiatrists, psychologists, nurses, dieticians and other professionals.

The sort of treatments that may be offered include the following:

Psychological ('talking') treatments such as cognitive behavioural therapy.

This will help to look at the reasons why you developed anorexia and to address your fears regarding weight gain or your body image.

Talking treatments take time and usually require regular sessions over several months.

Treatment of any physical or teeth problems that may occur - This may include taking potassium supplements, dental care, and hopefully not using laxatives.

There are also a number of self help books and manuals available to help you out of the condition. You can also take help from various self-help groups and associations concerned with this eating disorder.

Do I need to be admitted in hospital for all this?

No! In most cases, people with anorexia are treated as an outpatient.

However, you wud need to visit hospital regularly to so that we can check your wieght and assess the progress.

However, if we think that you are very underweight, you might need to be admitted.

Do people actually recover from this? Will I recover?

Well, Miss Smith I understand your concern. However I do assure you that if you start improving upon your eating habits and reach the desired weight required for your age, you will hardly have any problems in recovery. About 4 in 10 recover fully. and another, 3-4 in 10 improve, but continue to have some eating problems, but not 'full blown' anorexia.

Will I die because of this?

Well, this is a potentially dangerous condition and about 1 in 20 can die from causes related to anorexia. Usual causes of death include infections, dehydration, blood chemistry problems and depression. So you should not take this lightly. Be rest assured though, we all are here to help you.

Do let me know if you have any more doubts regarding the condition. Thanks for listening.

during the counselling it is very important to assess wether the patient has insight into her problem. lack of insight is an indication for urgent referral to EDU( eating disorder unit).
also to try and get family support.
I'm sure your family will support you during the entire treatment process. would you like me to have a word with them.
any comments?
please correct if wrong.
