A.SPLIT THE PLASTER...

You split the plaster when there is an increase in swelling or you suspect compartment syndrome.

You decompress the compartments when the patient has developed compartment syndrome.

You advise elevation of the leg following application of plaster in fracture of long bones to reduce swelling and thus prevent the complication of compt. syndrome.

if you think at the SHO or medical officer level your job is to split the plaster, but the idea here is to elicit the best work that you cando at your level at that point of time- clearly it is the plaster splitting or removal.of course the ideal way to treat a compartment syndrome is to recognize it first. and the prompt action should be emergency decompression of the facial compartments which should save the leg of the patient. but this requires an expert help. hence i feel splitting the plaster is the best answer at sho level and inform the orthopaedic surgeon and to post the case for emergency decompression
