ans is OCP

1st step in polymenorrhea is Medical treatment with ocp...if it fails then D&C...if it fails last step is hysterectomy

I agree that in a 42 yr old woman a D&C is more important tha just starting her on OCPs.

90%of DUB is due to anovulation. During anovulation cycles, failure of ovulation results of lask of progesterone, thus endometrium is exposed to prolonged unopposed stimulation. This results in endometrial overgrowth with estrogen-dependent breakthrough bleeding. Also, 10% of DUB is due to dysfunction of corpus luteum and inadequate progesterone production or atrophic (i.e.2 degree of OCP) endometrium.

Therefore, treatment with medroxyprogestrone acetate (Provera) 5-10 mg od on first 10-14 days of eacn month will be adequate.
