1) Treatment of choice in infantile spasm is

One answer only.

a) Phenobarbitone
b) Clonazepam
c) ACTH
d) Phenytoin



answers been marked wrong.
everybody knows that antiepileptics are not effective in west sydrome.
TOC is corticosteroids or ACTH.

or has some new research restored the efficacy of phenytoin here??? icon_surprised.gif

ACTH is definitely the preferred treatment for infantile spasm.

as the theory goes like overproduction of CRH (in response to diff 'stress' an infant's brain is subjected to),which acts as a neurotransmitter and excites the neurons all arnd and cause the infantile spasms.

as the infant grows,the no. of CRH receptors decrease and spasms automatically gets 'cured'.

till this stg ACTH is used to decrease CRH secretion by FEEDBACK inhibition.

other drugs used are clonazepam.nitrazepam & vigabatrin.

not phenytoin anyway!

doc girl is right.ACTH is the ans to the above ques

But there is some controversy whether ACTH or vigabatrin is the DOC of infantile spasm.?anyone having a definite ans to the above ques plzz reply

when in doubt stick to the established conventional treatments of choice,unless you find a standard textbook mentioning a differing opinion.
for the MCQs in paeds i stick to ghai so i'll go for pred & ACTH.

but do note Drug of choice in INFANTILE SPASM WITH Tuberous sclerosis is Vigabatrin..sorry I dont have a reference but will try get one
