29 year old woman with a history of mitral valve prolapse presents to A&E with a four days of fevers, myalgia and a sore throat. She has tender cervical lymphadenopathy and a short mid systolic murmur. Examination is otherwise normal.

Two weeks after returning from Safari with his family, a 16 year old boy develops a fever, headache and arthralgia. He has a small ulcer on the inner arm. The following morning he develops a widespread purpuric rash.
Hb 107 g/l
Bilirubin 13 µmol/l
WBC 6.2 x 109/l
LDH 672 iu/l
Plts 122 x 109/l
Fibrinogen 1.2 g/l

Which test would you arrange :-

A. Rash biopsy and immunofluorescent staining

I think the woman in Q1 is suffering from glandular fever, but I'm not sure whether throat swab could yield EBV.

Q2: E. Blood C/ST is always important in sepsis!
