1) The right time to apply for extension is 2 weeks before expiry.
2) Either attachment letter or MRCP exam (although the Royal College would send the confirmation two weeks before the exam), so to be on the safe side one needs to show a reason to stay in the next month, like it would be better to show an attachment letter that starts in a month after expiry. A months attachment that starts in Nov should suffice.
3) The appropriate form and fee, the reason of extension, and documentary evidence that you have enough £££ to fund your stay. A bank statement with a letter from the sponsor would do. (you dont need to have your own account).

Read what it says on the form, you HAVE to provide evidence that you have sufficient funds druing your stay, so if you don't show any evidence of funds there is a very high possibility that your application will be refused!! Call the home office to make sure!!

I can't give you exact answers for that, because I don't know which option is better.

What I said earlier is clearly stated in the FLR(O) application form, that if you are applying for extension of a visitor visa, then you have to provide evidence that you have sufficient funds for the visa duration, and since this is a requirement then if you don't provide it with your documents you are risking getting your application refused.

I'm not sure about overseas banks, if you have a credit card then I think this would be ok, or if the bank has a branch here in the UK. If you want to take a letter from a sponsor instead, then I think he should at least have a couple of thousand pounds in the bank statement that you will be taking to the home office. Once again I cannot say that this is guaranteed information, this is to the best of my knowledge and I'm saying it for the sole intention of helpin others. I have no other interest whatsoever.

The best thing to do would be to call the home office, and asking them these questions, to make sure that when you go or send your application for extension you have everything they need. Give them a call and see what they tell you.

All the best.
