most probable cause is duodenal obstruction

this is actually a case of Wilkie's disease or arteriomesentric compression.
also known as cast syndrome

occurs in patients immobilized in a whole body cast(as in spinal cast)
and also in acute loss of fat. icon_eek.gif

the third part of duodenum gets compressed between the vertebral column and superior mesentric vessels. 8)

pt presents with features of GOO(gastric outlet obstruction) with bilious vomiting. 8)

treatment of choice is duodenojejunostomy. icon_surprised.gif

any atheroma,which has to cause a pain in an heart, has to be vulnerable enuf to break up n attract platelets n other stuff to grow on it,n dat too fast,so dat the heart doesnt gets enuf time to develop contacts(collaterals),and it aches!!!! icon_lol.gif

a thick fibrous cap wud only make the atheroma strong headed,n thus less vulnerable to break,n it continues to grow,though very slowly,so dat even if the critical limit of lumen obstruction is crossed,the heart wud have developed enuf collaterals,by then!so dat it is unlikely for this to cause pain! 8)

The artery of wilkie is the supraduodenal artery which arises from the gastroduodenal artery in the majority of cases and less commonly from the common hepatic artery, supplies both the anterior and posterior surfaces of the first part of the duodenum.

Correct me if I'm wrong, but this appears to be a case of balanced translocation, not duplication, so the chance of a down$ baby will be nil
