seen in 50% of patients with primary syphilis and about 90% of patients with secondary syphilis; Due to inflammatory cytokines released after destruction of organism with antibiotic; within 24 hours

Also seen in Lyme disease, Brucellosis, Leptospirosis and leprosy treatment

Give steroids to decrease symptoms before and with treatment
