Aim USMLE Step 1

Clinical electives and toefl
I have not given step yet . Is toefl score of 101 okay for applying to electives ?? What are my chances of acceptance in colleges like mount Sinai , Cornell, Yale, Harvard ?? Reading :26, Listening : 24, Speaking : 27, writing:24. Can my speaking score help me to get electives?
