At 28 weeks gestation, amniocentesis reveals a AOD 450 of 0.20 which is at the top of third zone of the liley curve. The most appropriate management of such a case is:
1. Immediate delivery.
2. Intrauterine transfusion.
3. Repeat Amniocentesis after 1 week,

Amniotic fluid bilirubin values are at best an indirect evaluation of the state of disease. No serious complications are likely to occur when these values fall in the lower or lower mid-zones of the Liley chart. When bilirubin values fall in the upper midzone or upper zone, cordocentesis should be performed to prevent overreaction or to perform an INTRA UTERINE TRANSFUSION.

a chart that uses the spectrographic measurement

of amniotic fluid bilirubin levels plotted against

gestational age to estimate the severity of fetal

hemolysis resulting from Rh isoimmunization.


The chart is divided into three zones; a

measurement falling in zone 1 indicates no disease

or mild disease, while one falling in zone 3

indicates severe disease with impending fetal

death.

the guide says that it lies on the top of the second zone!!!!

and says third zone may be the misprint!!!!

now just a whole word wud be misprined or the numerical!!!

anywyas now we know th management in all the zones!!!!

if

zone 1 = repeat amniocentesis at 2-4 wks

lower zone 2 = repeat amniocentesis at 1-2 wks

upper zone 2 and zone 3 = intrauterine transfusion
or
delivery depending on gestational age

zone 1 = 38 - 40 wks

lower zone 2 = 36 - 38 wks

upper zone 2 and zone 3 = 34 wks
