WHEN PAP SMEAR REPORT IN CASE OF POSTMENOPAUSAL WOMAN SAYS-THERE R LARGE NO OF INFLAMATORY CELLS MASKING EPITHELIAL COMPONENT WHICH CONSIST OF FEW SCATTERED SUPERFICIAL SQUAMOUS CELLS.THE CELLS HAVE SMALL NUCLEUS
TRICHOMONAS VAGINALIS IS PRESENT
MONILIASIS ABSENT
MATURATION IN \DEX CANT BE GIVEN BCOZ OF HEAVY INFLAMMATORY EXUDATE
MALIGNANT CELLS R ABSENT
MY QUESTIONS HERE R
IS THERE NEED FOR REPEAT PAP SMEAR AS INFLAMATORY EXUDATE IS MASKING THE EPITHELIAL COMPONENT?IF YES THEN WHEN TYHAT IS AFTER THE INFECTION GETS CLEARED
R THESE FEW SCATTERED SQUAMOUS CELLS FROM VAGINA OR CAN THEY BE MALIGNANT AND NEED REEVALUATION
FOR TRICHOMONAS PT IS NOT TOLERATING METRONIDAZOLE AND ITS COGENERS SO CAN CLOTRIMAZOLE OR ANY OTHER VAGINAL PESSARY BE USEFUL?

that sure i a long q .i lost track midway of the q icon_eek.gif
methinks the pap smear has to be repeated.the very fact that it says trichomoniasis is present is the reason.till then anything regarding commenting about malignant cells should be avoided.
yup clotri pessary will be tolerated by her.
is the patient alcoholic.she is a woman so unlikely .but stll id askfor the history coz shes not tolerating metro icon_question.gif

no pt not alcoholic
so many people cant tolerate metronidazole bcoz of nausea n vomitting n metallic taste

this question has 3 components basically
1)in iflammatory smears answer is treat infection and repeat smear

2)regarding squamous cellsthese cells r r ASCUS
ASCUS stands for "Atypical Squamous Cells of Undetermined Significance". This designation means that the smear showed cells that were not typical squamo-columnar cervical cells or endocervical glandular cells. However, the abnormality is not sufficient to constitute "dysplasia." The management is to try to discover if the abnormality is "significant" by seeing what happens over time. A re-evaluation in 6 months by pap smear is sufficient.
3)tablets taken with meals
i/v metronodazole may work (Not sure )
