INTERCOLLEGIATE COMMITTEE FOR BASIC SURGICAL TRAINING

CERTIFICATE OF COMPLETION OF BASIC SURGICAL TRAINING (CCBST)
The Intercollegiate Committee for Basic Surgical Training (ICBST) was established by the Surgical Royal Colleges of Great Britain and Ireland to put in place a standards based curriculum framework and quality assurance process for basic surgical training that would support trainees for:

a) the provision of service in the generality of surgery, under supervision, as part of a surgical team in an NHS or equivalent hospital;
b) entry to a higher specialist training programme in Great Britain and Ireland .

The Surgical Royal Colleges, jointly with the Specialist Training Authority (STA) and, from 30 September 2005, the Postgraduate Medical Education and Training Board

will determine the nature of the training programmes for each of the recognised surgical specialties through the auspices of the Joint Committee on Higher Surgical Training (JCHST). Trainees successfully completing these programmes are eligible for a Certificate of Completion of Specialist Training (CCST)* and entry to the Specialist Registers of the General Medical Council (GMC) of Great Britain and the Irish Medical Council.

However trainees embarking on these specialty programmes must have a minimum standard of knowledge and experience of basic surgical practice in order to progress and these regulations specify the nature and extent of this requirement. Trainees who satisfy the requirements will be issued with a Certificate of Completion of Basic Surgical Training (CCBST). With effect from 30 December 2005, JCHST has determined that the CCBST will be a mandatory requirement for entry to a specialty training programme leading to the award of the CCST. The CCBST has been introduced following a decision by the Surgical Royal Colleges of Great Britain and Ireland to remove the mandatory training requirements for entry to the Intercollegiate MRCS . The CCBST will also provide a route to higher surgical training (type 1) for trainees who have completed basic surgical training which has not been approved by one of the Surgical Royal Colleges of Great Britain and Ireland . It will allow them to have such training assessed through a consistent and transparent intercollegiate process to determine whether the educational component of the training is equivalent to that of inspected and approved posts.

The introduction of the CCBST forms an amendment to the current guidance set out in the guide to Specialist Registrar training (The Orange Book)
and supersedes information provided in the guide to Higher Specialist Training in the United Kingdom and Ireland (The Pink Book)

for further information. Individual Colleges will provide a service for their current Members, Associate Fellows and Fellows who wish to be awarded the CCBST in that they will confirm the evidence and issue a certificate.

Applicants who have undertaken some or none of their training in a programme or posts approved by the Surgical Royal Colleges of Great Britain and Ireland will require submitting an application under Section B of the regulations to the ICBST Office. The website will provide information regarding the regulations, advice to applicants, the assessment criteria and application forms.

In the meantime, ICBST are giving trainees an opportunity to Register their interest in applying for a CCBST. This will give the Committee an opportunity to gauge the number of applicants for processing purposes. It is anticipated that trainees will be able to access and submit their application form for a CCBST from 31 October 2005 and the forms be processed from 30 December 2005.

How many specialities you need to work before getting ccbst

The intercollegiate MRCS is being with withdrawn this year and the training will be modified in accordance with the new MMC programme. As a result the training for CCBST will also be withdrawn and replaced with something else (the Royal Colleges don't know what at the moment). Because the plan is to go straight into run-through training after the Foundation programme, the CCBST will no longer be the form of assessment.

There is very little information about this on the RCS(Eng) website. The best information available at the moment is on the RCS (Edin) website - there is a document called "becoming a surgeon" that outlines briefly the new proposals.

Full information about the withdrawal of MRCS and CCBST is available on the news sections of the RCS(Eng) website.
