rituximab:
(1) Category :->The RITUXAN® (Rituximab) antibody is a genetically engineered chimeric murine/human monoclonal antibody directed against the CD20 antigen found on the surface of normal and malignant B lymphocytes. The antibody is an IgG1 kappa immunoglobulin containing murine light- and heavy-chain variable region sequences and human constant region sequences

(2) Mode OF action :-->Rituximab binds specifically to the antigen CD20 (human B lymphocyte restricted differentiation antigen, Bp35),

(3) Indications :--->
RITUXAN® (Rituximab) is indicated for the treatment of patients with relapsed or refractory, low-grade or follicular, CD20-positive, B-cell non-Hodgkin's lymphoma

(4) R.O.A :--->Parenteral
Injection (U.S.)


(5) preg,lactation/sp.precautions/side effects..... ::-=>Fatal Infusion Reactions,Tumor Lysis Syndrome (TLS): ,Severe Mucocutaneous Reactions,Hepatitis B Reactivation with Related Fulminant Hepatitis,Hypersensitivity Reactions,serious or life-threatening cardiac arrhythmias,angina,severe renal toxicity including acute renal failure requiring dialysis

Pregnancy Category C:
Nursing Mothers: It is not known whether RITUXAN is excreted in human milk. Because human IgG is excreted in human milk and the potential for absorption and immunosuppression in the infant is unknown, women should be advised to discontinue nursing until circulating drug levels are no longer detectable.
Pediatric Use: The safety and effectiveness of RITUXAN in pediatric patients have not been established


Initial Therapy:
RITUXAN is given at 375 mg/m2 IV infusion once weekly for 4 or 8 doses.

Retreatment Therapy:
Patients who subsequently develop progressive disease may be safely retreated with RITUXAN 375 mg/m2 IV infusion once weekly for 4 doses. Currently there are limited data concerning more than 2 courses

Antithrombotic
Monoclonal antibody (antithrombotic)
Platelet aggregation inhibitor

Abciximab (c7E3) is a chimeric mouse-human monoclonal antibody directed against the GP IIb/IIIa receptor;

(2) Mode OF action :---->its mechanism of action is thought to be steric hindrance of the GP IIb/IIIa receptor itself as opposed to direct binding to the RGD binding site of the receptor. It also inhibits the vitronectin (av b3)receptor, which mediates both platelet coagulation as well as endothelial and vascular smooth muscle cell proliferation. The clinical significance of the vitronectin-receptor blockade is currently under study.



(3) Indications :---->Abciximab is indicated as an adjunct to percutaneous coronary intervention for the prevention of cardiac ischemic complications

• in patients undergoing percutaneous coronary intervention

• in patients with unstable angina not responding to conventional medical therapy when percutaneous coronary intervention is planned within 24 hours

The safety and efficacy of Abciximab have only been investigated with concomitant administration of heparin and aspirin as described in CLINICAL STUDIES

Traztuzumab: anti HER2 antibody

used in breast cancer with HER2 +vitiy



Stage IV

The intentions of treatment are palliation of symptoms, improvement in quality of life, and prolonged survival
Treatment is based on tamoxifen and chemotherapy
Trastuzumab may be combined with chemotherapy if patient is HER-2/neu-positive
Surgery and radiotherapy may be applicable in symptomatic metastasis
Bisphosphonates (pamidronate) can be used to reduce skeletal morbidity in patients with bone metastases
High-dose chemotherapy with stem cell support is only indicated in clinical trials

harry says........The lessons were learned; humanized antibodies against the CD20 molecule expressed on B cell lymphomas (rituximab) and against the HER-2/neu receptor overexpressed on epithelial cancers, especially breast cancer (herceptin), have become reliable tools in the oncologists armamentarium. Each used alone can cause tumor regression (rituximab > herceptin), and both appear to potentiate the effects of combination chemotherapy given just after antibody administration. It is likely that other antibodies against other important tumor targets will be available soon.

also..........EBV-specific T cells generated from the donor have been used experimentally to prevent and to treat EBV-LPD in the allogeneic recipient. Some studies indicate that EBV-LPD can be treated with antibodies to B cell surface antigens. Use of an anti-CD20 monoclonal antibody (Rituximab) to treat B cell lymphomas that express this surface protein has elicited some dramatic responses. Studies are in progress to assess efficacy in EBV-LPD, in which the involved B cells commonly bear CD20.
