best substitute of esophagus after esophagectomy is...

1..stomach
2..jejunum..
3..left colon...
4..right colon..

answer given is stomach nd reason given is ease of mobilisation...

at one discussion of test, it was told that stomach is preffered bcoz of ease of mobilisation but 'BEST' substitute is colon.........plz comment...

Stomach appears to be the conduit of choice bcos of ease in mobilisation and its ample vascular supply


Colon is used if the patient has undergone a partial or total gastrectomy previously , or if the tumor involves the stomach to preclude a 5cm margin


Jejunal loops can also be used , but limited vascular supply restricts its mobility

reference schwartz 7/e
PARTIAL ESOPHAGECTOMY=jejunum is best
because jejunum, unlike stomach and colon, is a dynamic conduit so helps in propulsion of bolus of food.

TOTAL ESOPHAGECTOMY= after reading schwartz
i felt colon is better than stomach. various advantages of colon over stomach is given
including less post op complications.
only imp. adv. of stomach over colon is ease of surgery.

since neither partial nor total is mentioned in given in question, we shall take it as total because it is much more frequently done than partial.

left colon

cushieri textbook of surgery says left colon is preferred over right one for substitute after esophagectomy

if they specifically ask PARTIAL esophagectomy
then answer is clearly given as JEJUNUM

for TOTAL its upto you
i'll go for colon
