A 30 yr old male had a heart transplant and is being maintained on immunosuppressant,Cyclosporin.He developed candida infection and is treated with Ketoconazole.Why is this poor therapy?
a]ketoconazole is not effective against candida
b]ketoconazole reacts with cyclosporin to inactivate it.
c]ketoconazole has a potential for cardiotoxicity
d]ketoconazole inhibits cytochrome p 450 that inactivate cyclosporin
e]ketoconazole cause gynaecomastia and decreased libido in males

Itraconazole, Ketoconazole, and high doses of Fluconazole have been reported to inhibit the metabolism of cyclosporine; this may increase the plasma concentration of cyclosporine to potentially toxic levels; a few studies have not found a significant interaction between fluconazole and cyclosporine; however, plasma cyclosporine concentrations should be monitored carefully in patients receiving any of the azole antifungals; the dose of cyclosporine may need to be reduced; it is currently recommended that the dose of cyclosporine be reduced by 50% when itraconazole is started

Drada your choices are confusing, ketoconazole inhibits metabolism of cycloserine, not the cycloserine itself.
