Why not sparfloxacin, I think both trovafloxacin and sparfloxacin do not need kidney for excretion!!

Trovafloxacin:

About 50% of an oral dose is excreted unchanged.
~43% ---->feces
~6% ------> urine

Renal impairment doesn't affect the pharmacokinetics and its serum concentrations are not significantly altered in patients with severe renal insufficiency (creatinine clearance <20 ml/min), including patients on hemodialysis.

Sparfloxacin:

Excreted in both the feces (50%) and urine (50%). ~ 10% of an orally admin. dose is excreted in urine as unchanged drug in patients with normal renal function.

Adequate hydration of patients should be maintained to prevent the formation of highly concentrated urine.

Necessary to admin. it with caution in the presence of renal insufficiency.
Careful clinical observation and appropriate laboratory studies should be performed prior to and during therapy since its elimination may be reduced.


Adjustment of the dosage regimen is necessary for patients with impaired renal function (creatinine clearance <50 ml/min)

So best option would be trovafloxacin.

Thanks Kanna for your effort! I knew trovafloxacin was the single best answer but just wanted to make a point that sparfloxacin too can be given in renal failure as it does not depend solely on kidney for excretion.
