FAQ: Some US visa answers for IMG

1. With j1 img is obligated to return to his country after completing his residency program. Is it possible to change J1 visa , during residency program , to the other tipe of visas or some other residency status ?
Not during your residency. Following training, include fellowship here, you will need to find an employer who will sponsor you for permanent residency or your green card (which are now pink). But it is following the training. You should start looking for the waiver position about half way through your training, giving about 18 months to 2 years to complete the paperwork process.
You are correct, with H1b, the visa holder has no obligation of returning to your home country. You still have to find an employer who will sponsor the change of the H1B to the green card - don't assume that it will the hospital that gives you the H visa. The hospital that I work for will sponsor an H1B visa, but will not sponsor to change it to permanent residency. Also, you should know that most fellowships will not sponsor an H1B visa - so if you want to subspecialize, you would be better off getting your J1 visa and looking for a waiver prior to finishing your fellowship.
2. What requirements should the img satisfy in order to obtain this visa ( I know that he has to have all three USMLE
test) ?
To obtain a H1B visa, you must have USMLE
Steps 1, 2 and 3. You must have current ECFMG certification. And you must have the training license for the state that you will be working in. And you must have all this in your hands prior to submitting a complete application.
3. When does this visa expires?
You can remain in H1B status for a maximum of six years.
4. Is it difficult to change this visa later to permanent resident status ?
Not if you find an employer willing to sponsor you.
5. How large are costs of issuing this visa for Img ?
The J1 visa will cost you about $130 dollars. This doesn't count the cost of the exams and the CSA. The H1B doesn't cost as much - but most programs will strongly encourage you to pay for the premium processing. And it does work. And, a little known fact, it is written into the rules of the H1B visa, that the employer must pay for the fees for this visa.

hi,
that ws a goldmine of critical info! thx a lot. i heard that the rejection rate for j1 is very high...so most people hve advised me to go for h1b visa...is it so? frankly i personally wld "ve no probs wth a j1 "cuz at this time JUST getting a residency wld be good enough 4 me. but i "ve also heard that, the chance of getting a residency is also much better if we "ve a h1b compared to j1("cuz step 1 passing is reqd then). is it true tht 4 landing up wth a good residency prog step 3 passing/h1b is essential/advisable?
wld be grateful for your reply.

wanted to clarify some points for you.
tona - a couple of quick answers for you

* absolutely not. this visa is sponsored by ecfmg. it is a fairly straight forward situation.
"so most people hve advised me to go for h1b visa...is it so?"
* most pple prefer H1B because that is the route towards continuing to work there, greencard, citizenship etc.
* H1B is more difficult to get as everyone wants it.
it is a visa sponsored by the residency program. obviously programs dont want to burden themselves with getting visas for students. thats why they prefer citizens, greencard holders or j1.
* guess what?! the program has to pay for your H1B visa while you have to pay for J1. in reality many programs insist that their candidates pay for the H1B premium processing (a cool $1000).
* many top residency programs dont sponsor H1B.
* H1B's are easier in community hospitals
* some specialties like peds are more likely to give J1

* great thing is its considered easier to pursue your fellowship on a J1 (its also possible on H1B)

didnt follow you there...
* you can get either visa only after selection to a residency program not before. step 3 is a prerequisite for getting H1b but not J1.

* for all the above reasons pple prefer H1B. to repeat
quite a few top programs dont give H1B
additionally
* H1B can be extended to beyond 6 years under certain circumstances (i'll go into that later)
* if you have a j1 but want to stay on in US (without returning to home country) you can apply for J1 waiver (work in a US underserved area for 3 yrs)

Dear , my h1 visa has been approved last month but have not got my clearance back yet . My residency is going to start on 23rd june. I am much tense now. Can you give me any advice regarding speeding up the clearance process.
