O & G : Hysterosalpingogram

Hysterosalpingogram is useful in the diagnosis of all the following conditions EXCEPT
1. Cervical incompetence
2. Tuberculous endometritis
3. Asherman's syndrome
4. Scar integrity in previous LSCS
This Q was asked in the TNPSC
exam held on 16.10.2005.When I referred I found that some sort of radio opaque dye investigation is used in the diagnosis of all these 4 conditions.However in SHAW I read that in 2 the appearance is beaded and in another place it is written that HSG is C.I in 2 because of the risk of disseminating infection.Also for 1 and 4 the procedure done is hysterocervicography and hysterography.Are these the same as HSG??So which one??

hi!
the answer is tuberculous endometritis because of the same reason u said ie disseminating infection.
the beaded appearance is seen in TB fallopian tubes.
in rest three it can be used for diagnosis!

Hi,
i think ans is 1. Aditi, though HSG is containdicated in 2, it is still USEFUL as what is mentioned in the question.

A prospective study was carried out on 230 consecutive hysterosalpingograms (HSGs), in which a variety of techniques was used. The primary site of investigation was tubal, uterine and cervical in 76.5, 7.8 and 15.7% of cases respectively. The highest incidence of abnormal findings was obtained in the investigation of secondary infertility, especially following tubal surgery. Twenty-five women had a laparoscopy and dye insufflation within nine months of their HSG, and fully concordant results were found in 17 cases. The disposable cervical cup method of hysterosalpingography offered a precise means of quantifying the internal cervical os diameter. The mean internal os diameter in those women referred with a past history of mid-trimester abortion was significantly greater than that of the group with secondary infertility (P less than .05). A diameter of 0.7 cm is suggested as a practical cut-off point for the radiological diagnosis of cervical incompetence.

things contraindicated cant be done!
may i know how wud it be useful for it?
as i said before beaded appearance is for TB salpinx!
i remember-
i had referred something std , when i wrote this answer, as ample amount of time was available then.....

It's given in Shaw that it's c/i if you KNOW!
The q is not it is USED, but it's USEFUL..
Yes, it's clearly useful in diagnosis, but if u know already that pt is having tb endometritis, then don't do HSG..
Not much heard for its use for integrity of scar in LSCS?
