in which of teh following conditions wld maternal serum AFP values be the HIGHEST???

1) downs syndr

2) omphalocele

3) gastroschisis

4) spina bifida occulta

The answer is gastrochisis.
Gastrochisis is bigger abdominal wall defect as compared to omphalocele. There is no membrane covering the herniating organs.
It has higher levels of alpha fetoprotein than omphalocele.
The other two options can be easily ruled out as in downs afp goes down and in spinabifida occulta the defect is closed on surface.
