what is the best managment in a pt. with ca base of tongue and nonpalpable nodes?
1.unilateral radical neck dissection
2.unilat. selective submandibular dissection
3.unilat. irradiation of neck nodes
4.bilat.irradiation of neck nodes
please give explanation/ref

hi !!!
when the nodes are not palpabe at all... a follow up should be done after doing the relevent surgical cure for the primary carcinoma...
. sometimes if teletherapy(in which cobalt 60 units is used) is given to patientssubmandibular nodes are also included in the field. reference is from .. das textbook of surgery...

Dear members,
1. Carcinoma of base of tongue spreads bilaterally
2. N-zero (nonpalpable) neck nodes r usually N+(micro-permeated) and manifest later in 60 % cases
3. If the primary is being treated by surgery then elective neck dissectoin bilaterally is performed upto level 3 in posterior oral cavity cancers
4. A simple followup is not adviseable coz N+ nodes coz later dissection is very difficult and cure rates are lower
5. Later radiotherapy is usually given if tumor burden is more
