3. staph aureus
4. trichomonas
plz let me know how is reactive arthritis different from other arthritis and whats its pathogenesis.

reactive arthritis comes under seronegative spondyloarthropathy which includes classic ankylosing spondylitis,psoriatic arthropathy,IBD related arthritis and reactive (reiters) arthritis.

reactive arthritis can be secondary to GI (dysentry-shigella,salmonella,yersenia,campylobacter etc) or genital infection(chlamydia, gonocoocal,mycoplasma AND etc).most of these individuals are predisposed by their genetic HLA b27 positivity which may itself be pathogenic in the causetion.

usually they are self limiting.but if recurrent-treat with DMARD.

there is a entity called POST STREPTOCOCCAL REACTIVE ARTHRITIS-WITH PRECEEDING streptococcal infecton- with elevated ASO titre- often confused with RHEUMATIC FEVER in the children/adolescent

right now in our rheumatology ward-we have a pt with PSRA .his ASO is 1660 and polyarthritis-no cause found for other obvious diseases.

RULE OUT OTHER causes by appropriate investigation .

trichomonas is rarely implicated(only case reports)

i will consider streptococus strongly among these choices ANSWER 2

Post-streptococcal reactive arthritis is a frequently observed entity that has an uncertain relationship with acute rheumatic fever. Although different criteria have been suggested for delineating these two conditions, none have been validated. In the absence of a clear definition, it has been impossible for investigators to carry out meaningful studies. Some children who were initially labeled as having post-streptococcal reactive arthritis later suffered from carditis and definite rheumatic fever with subsequent streptococcal infections. Some physicians now recommend that all children with post-streptococcal reactive arthritis receive penicillin prophylaxis
Factors from both the host and the pathogen are probably important in determining disease expression in poststreptococcal syndromes.
