i would go with "b) reduction in fat intake to 20%to 30% of calorie intake" because it will prevent atherosclerosis there by preventing extra strain on the heart due to narrowed blood vessel

besides the other 3[b,c,d option] ,other recmdation are-
avoid alchohol consumption.

and salt intake </= 5g daily.
