which is teh objective sign of identifying pulmonary plethora in a chest radiograph?

1) Diameter of main pulmonary artery> 16mm

2) Diameter of left pulmonary artery > 16mm

3) Diameter of descending rt pulmonary artery > 16mm

4) Diameter of descending left pulmonary artery > 16mm

option 3 --Diameter of descending rt pulmonary artery > 16mm is correct. asked a radiologist.

is this the pallas sign??

i think so--thats what we have read infinite times--
pallas sign--dilated rt pul artery--nd they just extended it a step further...

and a new thing appeared.

pulmonary plethora is seen in these conditions--

pulmonary edema.

left to rt cardiac shunts.


so the question is --weather pul edema is seen with pul embolism??

pg 202 harrison--gicves classification of pulmonary edema based on initiating mechanism--pul embolism causes pulmonary edema due to unknown initiating mechanism.

so i think --inference we can make out of this is --that this cd suggest any condition where pul edema is present.this cd well apply to pul embolism too.that way it cd be pallas sign--objective interpretation.
am i thinking rt trinity??

pul edema and dilation of pul artery coexist--thats what i cd make out.if wrong pls correct me.

I can give you only hints:

DD of pulmonary plethora:

any left to right shunt:

    atrial septal defect

    ventricular septal defect

    patent ductus arteriosus

venous congestion:
    acute pulmonary oedema


And Kumar & Clark (5th Ed p 717):

Pulmonary plethora results from left-to-right shunts ( eg-ASD or VSD). It's seen as a general increase in the vascularity of the lung fields and as an increase in the size of hilar vessels (eg- in the right lower lobe artery), which normally should not exceed 16 mm diametre.

referce sutton
there are 2 things
-1)pulm plethora-this term is used in context of CHD used to signify inc bld flow into pulm a.

2)pulm edema -this is exudation into the alveoli.pulm edema is seen in PTE becoz of hypoxia leading to epith damage

3)palla sign -as in the heart the chamber before the stenosed valve enlarges,similarly think of pulm a getting blocked by thrombus,and the upstraem part getting prominent.

hope that solves the confusion!
