also in kericterus what kind of bilirubin is seen conjugated or unconjugated...i think it should be conjugated...am i right/

ALSO UOBILINOGEN IS FOUND IN URINE IN KERNICTERUS[BUT NOT UROBILIN-FOUND IN HEPATOCELLULAR INJURY].
AM I RIGHT?

Every baby with hyperbilirubinemia should have a direct fraction measured at least once to rule out direct hyperbilirubinemia. Direct hyperbilirubinemia in the neonate is defined as a direct fraction greater than one third of total bilirubin and is always pathologic. Subsequently, if the hyperbilirubinemia is established as the indirect type, obtaining a direct fraction with every measurement is unnecessary unless the hyperbilirubinemia develops after the expected time frame for typical neonatal hyperbilirubinemia.

According to me Jaundice in newborn due to hypothyroidism is unconjugated hyperbilirubinemia.
and rightly said by Jayanta, its the same, i.e the unconjugated one that crosses the BBB. (Kernicterus) - lipophilic

Causes of Unconjugated/Pathological Hyperbilirubinemia
Mnemonic - M.D. In Bloody Hyperbilrubinemic Sepsis[/u]

Metabolic Errors - Galactosemia, Criggler Najjar Syndrome I & II

Drugs & Hormone - Hypothyroidism

Incompatabilities - Rh, ABO, other red cell antigen incompatabilities.

Blood causes- Hereditary Spherocytosis, G6PD, Pyruvate Kinase Deficiency, alpha -Thalassemias

Hepatic causes -
1. Extraparenchymal - Cholestasis, Alpha 1 AT deficiency
2. Parenchymal - Hyperalimentation, Hepatitis- Viral,CM

Sepsis- Txoplasmosis, CMV, Syphilis

U could help me adding to the list if i have forgotten any.

Just to refresh ourselfs on Unconjugated Hyperbilirubunemia; Pathological ~.

* Direct Bilirubin >2mg/dl
* Total Bilirubin >15mg/dl
* Increaswe in levels of bilirubin >0.5mg/dl/hr or 5mg/dl/24hr
* Clinical Jaundice in first 24 hours

hypothyroidism causes unconjugated jaundice...

causes of jaundice due to decreased Conjugation (Indirect Hyperbilirubinemia)

Familial nonhemolytic Jaundice
Crigler-Najjar Syndrome (Type 1 and type 2)
Gilbert's Disease (Gilbert Syndrome)
Hypothyroidism
Prematurity
Breast Milk Jaundice
