yea morphine is used for pain relief in myocardial infarction which is an hemodynamically unstable condition...pentazocine causes rise in BP...

pentazoscine causes tachycardia & rise in BP due to sympathetic stimulation.

guys KDT says that morphine causes fall in BP. It is used in MI coz it decreases the wrkload on heart & coz it shifts blood from pulmonary to systemic circuit. Nowhere is it mentioned that it can be used in a hemodynamically unstable patient.

even though the dont mention for fentanyl either, atleast it says fentanyl produces minimum cvs effects.

So amongst the options I think fentanyl is the best answer.according to me. Though if tramadol is there in the options, then that wld be the best choice, coz KDT clearly mentions (5th ed pg 426) that hemodynamic effects are minimal with tramadol.

pentazocine is contraindicated in MI 'coz pentazocine increase the cardiac oxygen demand and not becouse its c/i in hemodynamically unstable patients.
