Ogilvie s syndrome results from the denervationof the colon distal to the

1.hepatic flexure
2.mid transverse colon
3.splenic flexure
4.descendin colon

Ans.3.. Does anyone know what ogilvie's syndrome is ?Is it the same as cast syndrome..This question is from the 2006 comed k exam.

Ogilvies syndrome is acute colonic pseudo-obstruction...there is no mechanical obstruction but because of denervation the colon becomes hypotonic and non-propulsive...it can b seen even in medical conditions like myotonic dystrophy...dont know why it occurs only because denervation is distal to splenic flexure...possibly could be that the stools are more in a liquid to semi-solid state in the right colon and more solid in the left colon...isnt that why right colon disease present more with diarrhoea and left sided colon diseases present more with constipation??

Ogilvies syndrome presents with:
Abdominal pain (80%)
Nausea and vomiting (80%)
Obstipation (40%)
Fever (37%)

BAILEY 1201
COLONIC PSEUDO OBSTRUCTION
A/C[ OGILVIE SYNDROME] AND C/C
abd x-ray- colonic obstruction with marked caecal distension
confirmation- colonoscopy or a single contrast water soluble barium enema.
treatment- colonscopic decompression
recurrence in 25%
if recur may require further colonoscopy with simultaneous palcement of a flatus tube
when colonoscopy fails or is unavailable - a tube caecostomy required..
if still -ve, SUBTOTAL COLECTOMY AND ILEORECTAL ANASTOMOSIS

Cast syndrome is the superior mesenteric artery syndrome....that is compression of the 3rd part of duodenum...its a mechanical compression (true compression) causing a partial or complete intestinal obsruction..
