Hallux vagus is prominence of medial first metatarsal with lateral deveation of great toe.
Anatomical factors predisposing to hallux vagus are varus alignment of the first metatarsocuneiform joint. The medially directed first metatarsal widens the angle between1st and 2nd metatarsal beyond 5-8degreesThis is called metatasus varus. It provide mechanical advantage to adductor hallucis tendon insertiono the proximal phalanx of the great toe, with resulting lateral deviation of great toe.

The most significant causative factor is improper footwear.
