hi guys,
does any one know which is the most pereferable drug in treatment of hyperthyroidism in pregnancy, carbimazole or propyl thiouracil????

in OHCS p.157 carbimazole is mentined in control of hyperthyroidism in pregnancy and propyl thiouracil is prfered pstpartum as it's lss exrected in milk???
what do u think ?

i agreed v u dats gvn in OHCS but i read in merck manual n that says :-
"Radioactive iodine (131I), given to treat thyroid disease, can cross the placenta and destroy the fetus' thyroid gland or produce severe hypothyroidism. Triiodothyronine, propylthiouracil, and methimazole also cross the placenta and can cause fetal goiter. Saturated solution of potassium iodide--often used to prevent release of thyroid hormone from the thyroid gland when hyperthyroidism is clinically severe--should be used with great caution. It has been reported to cross the placenta and produce a large fetal goiter, leading to respiratory obstruction in the newborn. Methimazole may cause scalp defects (aplasia cutis) in newborns. Thus, propylthiouracil is the drug of choice for treating hyperthyroidism during pregnancy."
n emedicine also says PTU a DOC!
u may confirm it there or wait for some more replies

just go thrugh tripati antithyroid drugs chapter u will get the answer.hes given a table comparing carbimazole n propylthiouracil.
propylthiouracil does not cross the placenta hence safe in pregnancy,but carbimazole crosses the placenta hence contraindicated in pregnancy

It's given in Harri that it is the DOC in pregnancy..
The reasons are as follows:
1. It has relatively low transplacental transfer as compared to carbimazole & methimazole..
2. It also inhibits conversion of T4 to T3..
3. Carbimazole & Methimazole are associated with rare cases of foetal aplasia cutis & choanal atresia..
Any doubt now?
