Possible causes of dysphagia

Oropharyngeal

Anatomic
Post-cricoid cartilage web
Cervical osteophyte
Hypopharyngeal diverticulum
Head or neck tumor
Neurologic

Cerebrovascular accident
Poliomyelitis
Amyotrophic lateral sclerosis
Parkinson's disease
Cerebral tumor

Muscular
Myotonic dystrophy
Oculopharyngeal muscular dystrophy
Myasthenia gravis
Metabolic myopathy (eg, thyrotoxicosis)

Esophogeal

Neuromuscular
achalasia
Diffuse esophageal spasm
Scleroderma
Lower esophageal sphincter hypertension

Intrinsic obstructive lesion
Stricture
Tumor
Lower esophageal (ie, Schatzki's) ring
Foreign body

Extrinsic obstructive lesion
Enlarged left atrium
Enlarged aorta
Aberrant subclavius
Mediastinal mass (eg, thyroid, lymph nodes)





Sudden onset of symptoms may result from a stroke (oropharyngeal dysphagia) or food impaction (esophageal dysphagia). Rapid-onset dysphagia with solid foods and intermittent presentation that is not progressive suggest an esophageal ring. Intermittent nonprogressive or slowly progressive dysphagia suggests a benign cause, such as a motility disorder or a stable peptic esophageal stricture. A history of prolonged heartburn may suggest peptic esophageal stricture, neoplasm, or esophageal ring.

Dysphagia that progresses from solid to semisolid food or liquid in a brief period of time suggests esophageal stricture related to tumor. (Solid-food dysphagia consistently develops at a luminal diameter of <13 mm.)
