A full-term infant presents to your office at 7 days of age with bilateral purulent conjunctive discharge, erythema and swelling at the medial aspect of the eyes. Which one of the following is the most likely diagnosis?


1) Neisseria gonorrhoeae infection

2) herpes simplex infection

3) nasolacrimal duct inflammation due to Staphylococcus aureus

4) chemical irritation from eye oinment received at delivery

5) chlamydia infection


3 or 5? Chlamydia affects neonates on days 3-5. Dacryocystitis cause medial swelling. I think it is most likely 3. Any comments?

well if it is nasolac. duct inf, it is rather be unilateral. So I guess it is chlamydia?

because---gonococcal conjunctivitis; is bilateral, purulent; 2-5 days after birth

infection resulting from chlamydia may be watery early in the course and purulent later..........

Well friends, it seems chlamydial infection..

Chemical conjuctivitis appears after a few hours of dilivery & lasts for 48 hours..

Gonococcal after 2-4 days & it is a rare cause now..

Inclusion ophthalmia neonatorum appears b/w 5-14 days of delivery & is c/b acute mucopurulent conjunctivitis, it may be fankly purulent & hyperacute..

So I favour chlamydia trachomatis..

The Answer is definately Neisseria gonorrhoeae infection !!!

The infant is reporting on the 7th day so it cant be 'chlamydia infection' cuz it occurs in 2nd week of birth ....
and the discharge being purlent it cant be 'herpes simplex infection' (even though it do occurs between 5-7days of birth) but its discharge is Serous ...
cant be 'chemical irritation from eye oinment received at delivery' cuz it occurs between 2hrs - 1day of birth ....
and as for 'nasolacrimal duct inflammation due to Staphylococcus aureus' never heard of it in neonates ...

so my answer is Neisseria gonorrhoeae infection

FACTS go more in favor of gonococcal infection (purulent discharge nd all)....as far as the occurence is concerned gonococcal has substantial prevalence in developing countries.........so it is gonococcal 4 me
