carcinoma larynx....but now a days it is said that if total laryngectomy will be our further management depending upon the grading of tumor then it should be done immediately because study has shown that the cases of recurrences are more in tracheostomised patient than a non-tracheostomised patient.

one more pont is that high tracheostomy should be done in carcinoma cases so that stoma should not come in the way of incision in the neck i.e gluck sorensen's incision.
