Q.Lactic acidosis is seen in all except :

1.Metformin therapy

2.Circulatory failure

3.renal failure

4.Respiratory failure

in Harrison in list of lactic acidosis, renal failure is given but not resp. failure !

real confusion bet. renal & resp. failure ??

Lactic acidosis is seen with both met. & phen, BUT it is prominant with

phenformin.

so met. is ruled out !

causes of lactic acidosis

type A :d/t increase in plasma L- lactate 2ary to poor tissue perfusion :

shock , circulatory failure
severe anemia
mitochondrial enzy defects
mitochondrial enzy inhibitors like CO & CN-

type B : d./t anerobic d/o

malignancy
DM
renal or hepatic failure
severe inf ( cholera , malaria)
AIDS
seizures
drugs/toxins : biguanides, ethanol , methanol,INH ,AZT analogues & fructose

unrecognised bowel ischemia or infarction in a patient with atherosclerosis OR cardiac compensation receiving vasopressors

D- lactic acidosis : jejunoileal bypass or intestinal obstruction a/w increased AG & HYPERCHLOREMIA
