Answer would have been easier if asked which is the most common. All four are seen in CRS. The most common is PDA. Congenital heart disease including patent ductus arteriosus (PDA) and pulmonary artery stenosis is present in 50% of infants infected in the first 2 months of gestation. Cardiac defects and deafness occur in all infants infected during the first 10 weeks of pregnancy and deafness alone is noted in one third of those infected at 13-16 weeks of gestation. ASD and VSD are uncommon.

Look at this study by...

Rabindranath Tagore International Institute of Cardiac Sciences,
Kolkata

In February 2002, a registry for patients with congenital rubella syndrome (CRS) was started in the institute to coordinate comprehensive cardiac, ophthalmic and audiology services. Over the period of four months, 12 patients meeting the clinical criteria were encountered. The spectrum of congenital heart diseases was also noted as part of this study. PDA was present in 11/12 patients with one patient having a normal heart. VSD was present in 5 . Pulmonary valve stenosis was present in 3 patients, branch pulmonary artery stenosis was present in 2 and aortic stenosis in 1. Coarctation of the aorta was seen in 1 patient, ASD in 1 and bicuspid aortic valve in 1. Sensorineural deafness was present in 4 patients and cataract in 8 (4 bilateral and 4 unilateral).

most books give the answer as PDA
for all practical purposes option `d` is considered as the right answer.
