Avascular necrosis(avn) occurs due to disruption of blood supply to femoral head.

Intertrochanteric and subtrochanteric fractures are extracapsular fractures, blood supply to the femoral head is preserved and chances of avn are low

Intracapsular fractures cause disruption of blood supply to the femoral head by
1. direct disruption of blood supply
2. tamponading effect of the fracture hematoma

Although AVN can occur in undisplaced intracapsular fractures, displaced fractures (Garden 3 & 4) have been shown to be at higher risk for AVN
