THE ANS IS ASOM
In acute mastoiditis, the postauricular area is at first tender; swelling and erythema follow. The postauricular sulcus is blunted and the pinna may be displaced forward, but no subperiosteal abscess is present. Sagging of the superior posterior aspect of the external auditory canal may be seen on otomicroscopic examination. The tympanic membrane is usually thickened and inflamed but may be perforated with pulsatile mucopurulent otorrhea.

Yeah, pulsatile otorrhoea is in ASOM. When there is a perforation in the tympanic membrane and the pus oozes out, it pulsates with each heart beat. It's also called Lighthouse sign. (Ref : S.K.De ENT )
