i think fev1... what do others say!!!
thebest test is ratio of fev1 and vital capacity..
vital capacity in copd is normal but fev1 decreases.. so the ratio decreases.
opposite happens in restrictive lung diseases... vital capacity is decreased and fev1 remains normal...so the ratio increases...
and moreover in obstruction we are more concerned with the outflow ...so fev1 ....
but what about pefr....can anybody throw some light

Yes I agree.
FEV 1 in particular the ratio of FEV1/FVC
According to Kallenberg 4 stages of COPD are recognised.

Stage 0: defines patient at risk who have symptoms of chronic cough with sputum production
Stage I:mild,FEV1/FVC <70% but FEV1 > 80%
stage II:moderate FEV1 between 30 and 80 %
stage III:severe,FEV1 < 30%.cor pulmonale or respiratory failure.Exacerbations at this stage can be life threatening

no the ans is mefr ie mid exp flow rate. it is nothing but fev 25-75%.
Harrison's says fev1 is normal in early obstuctive disease and mefr is a better indicator of early disease. it is also called mmfr ie max mid exp flow rate
