Is TOEFL manadatory to work in the USA?
For me,I`m a doc and preparing for the USMLE .where is the site of TOEFL exam in the USMLE journey?
May IELTS be a substitute to TOEFL ?!

Yes it is mandatory. It is an exam to make sure you are proficient in the language of English. And that communicating and understanding english will not be a problem for you.
IELTS is to be given in UK, not USA>

TOEFL is not required for applications for residency training in US. what is required is score of USMLE exams.

So,can I understand from both of u that TOEFL is not required in residency training while it is a must to practice medicine after residency?
i`m really confused

no it is not required to practice in US either. but if you r planning to take university courses on some other subject........ say for instance public health, etc. then you need TOEFL score. also if you plan residency in Canada . hope this helps

May I ask other 2 questions regarding ur answer?:
1-if the residencey is part from university program(i mean training in university hospitals),should i clear the TOEFL then?
2-may I apply to residency training in Canada using the USMLE or i must go through CEE?

for residency training in US you donot need TOEFL .
for residency training in Canada you need that. i donot understand what you mean by CEE.
