Quick Scroll D/D of Black nails??? 09.07.07 (1 year ago) #1

hey ppl yesterday i saw a patient who actually came with a history of fall sideways leading to intertrochanteric fracture femur...but on general examination i found all finger nails to be black...she was haemodynamically stable but radial pulses felt and normal...all other perip pulses were normal...
can anyone guide me here...Make a list of D/D of Black nails???

D/D Black Nail

1. Subungual Hematoma
2. B12 deficiency(Pigmented Bands)
3. Cancer chemotherapeutic drug (Bleomycin, Cyclophosphamide, Methotrexate, Doxorubicin, 5-FU).
Other drugs(Sulfonamide,Antimalarials,Tetracycline,Phenytoin,Ketoconazole etc)
4. Subungual melanoma (The 2 most important signs of subungual melanoma are melanonychia striata (longitudinal brown to black pigmented streaks in the nail) and Hutchinson’s sign, which is the spread of brown or black pigment from the nail bed, nail matrix, or nail plate onto the adjacent cuticle or onto the proximal or the lateral nail fold.
5. SYSTEMIC CONDITIONS (Addison’s disease, Peutz-Jeghers syndrome, Hemosiderosis, Vitamin B12 deficiency, Porphyria, Radiation therapy Laugler-Hunziker syndrome)
6. MICROBIAL INFECTIONS (Onychomycosis)
7. Trauma
