Dr. I am.....feeling fatigue

I am sorry to hear that Can you please tell me since how long this is going on? What do you think is the reason for your fatigue? Is this happened all of sudden or it started gradually? Do you feel fatigue through out the day or in particular time of day? Wht do you think is it progressing? How is it affecting your life? Is there any thing that makes it better/ worse? Are you expereiencing any problems either physical or mental that you want to explore with me?
Ok Mr Over I am going to ask you certain questions that may or may not be associated with your fatigue Is that OK.
How is your mood?
What are the activities you enjoyed the most? Are you still able to enjoy them?
if mood and interests are affected explore MSIGECAPS
How is your sleep? Do you have any problem in initiating the sleep?
Do you wake in middle of night to catch the breath? any early morning awakenings? Any changes in work schedule? Do you feel fresh when you get up in the morning? ( Dont explore much if he has no problems in sleep)
Wt loss? Night sweats? Fever

Chest pain/ Shortness of breath/ Cough/ palpitations
Abdominal pain/ Bowel and urinary habits explore blood in stools or urine
joint pain/ swelling
edema of feet
adjustment to temperatures
headaches
muscle pain
numbness or weakness of any part of body
PMH of diabetes, HT, kidney diseases, liver diseases, heart diseases, lung diseases, cancers
hospitalizations, surgeries

Drugs: Diuretics, BEta blockers any other medications, drug withdrawl

allergies
diet
alcohol smoking
ill contacts
social history, support system, travel
