Sam is a 25 year old man living with his partner Jane. He presents to the emergency department saying that he " couldn’t go on any longer". He is employed at a bank but has not reported to work for a week , having called into say he is ill.
The history revealed that he was feeling sad and tearful most of the time and had been this way for almost 2 months. He could not sleep and often woke up early in the morning. His appetite was low and he had lost about 15 pounds. His energy and concentration were low and felt that he was not a good person. In fact many of the things that he said about himself were very self critical. He had never experienced a major depression before and was frightened by some suicidal thoughts he had recently.
