I'm an intern. i don't have my reg. no. yet. what should i fill in? there is no place for provisional reg no. i'm not able to download 4th and 5th pages. is anyone else facing this problem? r we supposed to mention exact dates of admission and leaving on pg 3? please help.

make a single d-d of 1000/- for both the fees.
i have send like this last yr.
also in the admission notice they have said that application shud b accompanied by a d-d (not d-ds) towards application fees & registration fees.....don't bother about it ...it is not scannable form.
