True story about a usmler
This is a true story of a man I know. Egyptian physician who got a humble academic score while in collage, however, underneath this calm looking face and those average scores lie a book worm person who can read any book at any field without delay. A polymath, multilingual person. He also had a special avidity towards medical books. He spent over 10 years reading in all branches of knowledge.
Finally when he reached 32 he found the love of his life and soon get married. Up till this he was working as a GP in rural Egyptian area, being tortured every day by his craving for practicing true medicine away from all the crab which surrounds him.
He appeared for USMLE step 2 ck and got 99%, then step 1 and got nearly the same degree, then he departed for the us for the CS exam. Guess what???!!!! He failed in his first attempt, he didn’t loose hope, and tried for the next time and failed again, I have seen his score profile and found his high score in the clinical skills, but the one which got him down was the interpersonal communication skills and the proficiency of English language speaking.
Now the dilemma lies in that… in his first attempts his grades in the communication skills and the English language skills was good, and in the second time it was bad!!!!! How come????!!!!

I am very sad about my friend, and I don’t know how to help him

I am sorry about your friend.
I want to tell him one thing that it may have something to do with the very manner of uttering his words due his being a non-native speaker and also may have to do with the region where he belongs i.e ; the Arab region.Arabs and other foreign speakers where english as a language is not spoken from the very begining of a child's life and thus the person maintains a very ununderstandable tone of language.
Ask him to listen to some audio tapes of good english speakers.
Ask him to record his words as he speaks english and then listen them again in order to understand where he gets them wrong.
May be he does not speak full words or he misses some syllables(parts ) of words.
May be he puts undue stress on some words and speaks others too much slowly/lowly.
Ask him to pause between sentences.
Ask him to always use words which are easier to speak usually.
Ask him to learn and use words which express thanks and which show gratitude like : thanks, ok, alright,etc.Let him incorporate into his daily speech.
Ask him to start speaking in english at home ,with his friends,relatives,etc.
Ask him to practice cases in front of others bearing in mind these guidelines.


As you say he failed second time as well and scored even lower than the first it may be due to the fact that he was more anxiuos second time due to his previous bad performance.
Ask him to be calm while interacting with the patient and ask him to be a little more humble in front of the patient

It is better to show to the patient that you are still a learner than to portray yourself as an extraordinary doctor.
