One failure overall in the USMLE exams will not have any substantial detrimental effect on your chances for residency.
I e.mailed to a lot of programs and have studied this year's residency program criteria for internal medicine . What i came to know is as follows:
1. Most programs need applicants to have minimum scores of >80 /196or 82/200 in both the steps.
2.The higher scores needed by any other programs are usually greater than or equal to 85/210 in both the steps.
3. If a person has scores of greater than 90/220 in both the steps then he is eligible for about >95% of the programs. But ,as the programs may also need some usce ,or do not sponsor for visas so it becomes an obstacle for the img's.
4. If any program has mentioned the so-called failure in their criteria, then mostly it is mentioned as more than one failure.
So, a single failure will not have any detrimental efect so long as one's scores are at least above the required minimum.
even if one can not apply to the very very few programs( which are usually not there) due to his one failure, then what about those candidates who can not apply to a vast majority of programs due to lack of USCE,US LORs or due to their scores being below the required minimum.
What about the California state's IMG unfriendly programs which are usually left out by most of the the IMGs ?
In a group of hundreds of programs how does it matter if we can not aplly to a few. Also, it is even natural that nobody applies to all the programs due to one's preferance for location,stipend,community/research based programs, couple's match, quality of a resident's life, etc.
so, simply think you have left such programs(if there are any) as you would have naturally left the many others due to your preferance.
I hope that you are satisfied by the respose.
Good luck to you,dear.

i think it matters but not that much.
Physician these criteria are just an eyewash otherwise everyone wud get calls easily. Dont know why they put these there when actually the criteria are much more stringent due to competition . hood look at it this way, whatever the criteria but u will be compared to ur peers. So u have to have a better cv then them.So if 80% pass in first atempt then those 80% wud seem better on this aspect to the program. but if u have other things going good it shud not make much of a difference.

One believes in only what one sees. That is why we take these criteria into consideration.
About their hidden criteria ,it may be a fact about some programs ,but can not be generalised at all.
If that would have been the case then no body would have ever got a residency at a score of say 80 in both steps which everyone knows does happen.
thanks

Hi Phicician..
Thanks for the information..
I am a graduate from india(2007)
I have a score of 90 percentile in Step 1.. n giving Ck on 26th June..
But the fact is that I failed CS in 2 attempts.. What do u think my chances are for Internal Medicine?
Should I continue with USMLE
or should I drop the plan?
Right now I am planning to go to Australia
after giving AMC MCQ exam.. as it has been made compulsary from this Juy'08..
Will experiance in Australia
be of any help? or do u want me to do some research in India itself?
Please help me.. I am n dilemma now.. Please.
Thanks in Advance..

Dear Physician,
Am an IMG, already done MD in clinical Pharmacology
. My step 1 score is 90 and step 2 CK score is 95.
but I got a fail in my first CS attempt.
Very much depressed, now planning to retake the exam in July last week after extensive preparation.
Whats ur advise for me? What are my prospects of getting residency. Am planning for 2009 match. After reading ur post, felt some relief
Please reply

dear ,
be brave and face the challenge.Don't resort to escapism.
You have two plus points:
(1)Step-1 score of 90.
(2)very recent graduate
Pass the step-2cs anyhow. The people in america know that img's can have problems in step-2cs.A failure in step-1 and step-2ck is far more detrimental than the USMLE
step-2cs failure.
Try for any coaching for the step-2cs.Take help .do not shy away.
Practice a lot.
Get a very high score on step-2ck>95.
Apply with a Step-3 done with a very high score of >95 as well.
If possible do an observership,if not an externship, dear.
Apply in a lot of programs particularly in new york, chicago, philadelphia, etc which r img friendly.
Write to me if u need help in USMLE
step-2cs
best regards
