there is sigificant association betwwen risk factor and disease
the disease is 2.3 times more comon in those having risk factor

there is significant association between risk factor and disease.
it can be interpretted as ratio of odds of developing the outcome(DISEASE)among those exposed to hypothetical risk factor to those who are not exposed.

there is association between the risk factor and disease.


OR means the likelihood of having a disease when the risk factor is present(comparing with the absence of that specific risk factor).Anything greater than 1 means association, however, greater than 2 is something that any research study will hope to prove.

You are right mdc, any odds ratio more than one tells us that the disease is "MORE LIKELY" to happen with the risk factor without giving the exact ratio.
So the answer should be:
there is sigificant association between risk factor and disease

You can't actually tell if OR=2.3 is significant or not. For that you will need to have either a p-value <0.05 or a confidence interval around 2.3 which doesn't include 1 (for example CI: 1.5-4.5). And we do not have this information in this case.
OR=2.3 means that the odds of having the disease is 2.3 times higher in exposed compared to unexposed.

"significant" there refers at 2.3 - the fact that is much higher than 1. Don't mix up with 'statistical significant' which indeed refers to p value.

So, if OR=2.3 and the CI is (0.9-3), will you say there is a significant association? Just because it is >1, it is not a significant OR. No study will ever report an OR with just a number without P-value or CI and refer to it as 'significant'. You can check this in any Epidemiology book.
