about Visa 422 (Medical Practitioner) - the sponsorship approval can be upto max. 4 years by Visa Department. the job contract is variable eg, if you have 1 year contract, the contract will be extended by you employer.

next: after sponsorship approval there is a good chance that you will get a 4 years VISA. but with change of employer, you have to inform and get a formal approval from Visa department.
