A 71 years old woman presents with 18 months history of gait ataxia, dysarthria and dysphagia. On examination there is downbeating nystagmus and slurred speech. There is past pointing in both upper limbs and a wide-based ataxic gait. Reflexes and sensation are normal. There is no wasting or fasciculations. Plantar response is flexor bilaterally.
What is the most likely diagnosis?
1 Botulism
2 Arnold-Chiari malformation.
3 Syringomyelia.
4 Cervical myelopathy.
5 Motor neuron disease.

Downbeating nystagmus indicating a craniomedullary junction Pathology . Arnold Chiari is one of the commonest causes.
