its simple!
to be eligible for fellowship u have to prove that u r a medical graduate............and if foreign graduate then for that u have to be ECFMG certified and
u get ECFMG certified only after Step-1,2,CS.
ur medical PG degree just count for proving that u have post grad medical qualification and can do fellowship training in that branch.
u cant practice in US after fellowship(and no residency) in USA.
because for US-practice u have to be 'board certified' in that field and to be 'board certified' u need to clear board exams and u give exams only when u r 'board eligible'.
u r 'board eligible' only when u do a US residency in that specialty.
i have heard ppl getting eligiblity to give board exams after doing fellowship, but u may have to do extra training (1-2 years) in that specialty after fellowship to get that. it varies from specialty ot specialty and program to program.
radiology fellowships are a commonly sought for by indians and i have heard ppl do get in.
but u need to be from good indian institutions(AIIMS
, pgi) as the HODs here have good exposure and contacts with their US counterparts, which matter a lot.
Contacts matter very much for fellowships.
because the race for fellowships among US residency trainees is also very tough, since fellowship options are limited.
so if u r interetsed, remember that u r in race with ur counterparts in a US residency
and check out how they prepare for fellowships during residency..............taking a year off for research, buttering the faculty, etc. etc.
best of luck, i hope i am understandable!

from a not so well known institution, u have to shine ureself.
doing case reports, research, etc.
publishing articles....
going for conferences to present ur research
preferable international conferences......
attend workshops.
in short build ur CV during ur postgrad years.
try to work on the branch u wanna do fellowship in. eg. if u want to go or cardio, then do all ur case reports , research related to cardio. try to show ur committment to the specialty, 'dont scatter ureself'!
but that doesnt mean u stop work in other fields, they get less preference than ur choice field.
something is always better than nothing.

Research fellowships in US don't need USMLE

I did Dermatology
MD in South Asia and I am now doing postdoctoral fellowship in University of California San Francisco.
I work as a research fellow. There are postdoctoral positions (you need to have MD or PhD from home country) that involve research, and these fellowships do not require USMLE
. You will be working in a clinical Department, but do not hold clinical responsibility. Yet you can see patients in the observer capacity with a senior doc if you are interested.
Clinical fellowships on the otherhand need USMLE
. It is not easy to get one anyway.
