A 73-year-old man has had an aortic aneurysm repair and requires artificial ventilation. His inspired oxygen is 50% (FiO2 0.5), tidal volume 800 mL, respiratory rate 8/minute.
An arterial blood gas determination shows a pH of 7.26, a PaO2 of 170 mm Hg and a PaCO2 of 52 mm Hg. Which one of the following is the best management option?
1) Decrease the tidal volume to 700 mL and decrease the FiO2
2) Decrease the tidal volume to 700 mL and leave the FiO2.
3) Increase the respiratory rate to 10/minute and increase the FiO2.
4) Increase the tidal volume to 1 L and decrease the FiO2.
5) Add sodium bicarbonate to the intravenous fluids.
