a 48 yrs old lady has irreagular vafinal bleeding for 2 months on pelvic exam no cervical lesions and pap thest shows no abnormal cells nest endometrial biopsy result come with microscopic evidence of endometrial hyperplasia.on abd u/s rt ovarian mass ..which of the following neoplasm is this women is most likely???

a) mature cyst teratoma
b)choriocarcinoma
c)sertoli-leyding cell tumor
d)fibrothecoma
e)krukenberg tumor

Yes,that's the differential.You can safely exclude choriocarcinoma,Sertoli-Leydig and Krukenberg.
I have considered a dermoid cyst in a premenopausal with DUB,which could be encountered in this clinical setting.Fibrothecomas are rarer(4% vs.10-15%teratomas), mostly postmenopausal, they have features of Meigs' and indeed, few of them display funtional activity(explaining endometrial hyperplasia).So,I guess Pathology will establish the diagnosis.
