following thyroidectomy young pt on maintanous iv dextrose and saline she develeped tetany and found serum calcium 2.20 mmol/L a blood PH of 7.48 and paco2 of 24 mmhg .most likeli diagnosis is.

1- metabolic alkalosis from dextrose and saline infusion
2-respiratory alkalosis
3-acute deficency of calcitonin
4-postoprative hypoparathyroidism
5-thyroid crisis

I agree after surgery for thyroid gland she developed tetany Calcium ia low. I will go for 4

Drs, this patient calcium level is in normal range(2.1-2.5). pH high, PaCO2 very low. Tetany can occur both in hypocalcemia and alkaloses. Thyroidectomy is a bate. We should always look at the QE1 question paranoid.
Respiratory alkalosis is an answer.
